I would like to redesign my Appointment Reminder Card. ONE SIDED ONLY! PLEASE DISPLAY A FLAT, STRAIGHT VIEW OF THE CARD ON AN ALL BLACK BACKGROUND. USE SAME FONT AND NO SMALLER THAN 8PT. 1. I like the current Fonts (linux Biolinum and Roboto) and colors. I am open to new fonts though. 2. I would like all the information to be on ONE SIDE only. Only the front, no back. 3. I need all of the information from the current Back and only the website from the current Front. 4. We now have multiple locations. So I would like my staff to be ale to easily check a check box to show the patient which location the appointment is at. Please include the two location information entitled "Location Info"
